Model Data Form

Use this model data form to submit some basic information about yourself to put together your photoshoot. This helps give me ideas about your looks and style. This information will be sent to me and I will contact you to discuss plans/concepts, locations and dates/times for your photoshoot.

OPEN CALLS ARE BY APPOINTMENT ONLY

Please bring the same photos as required by the “Photo Submission Guidelines” below and that you can leave with the agency.

Requirements:

Age 18+ (under 18 need parent/guardian permission)
Women: 5’4” to 6’
Men: 5’9” to 6’4”

Photo Submission Guidelines:
These photos should NOT be professionally taken or edited
Photos should be taken in front of plain background and outside in natural light if possible
Photos should be less than 500KB each and in .jpg format
Wear a swimsuit with no make-up and hair pulled back into a ponytail
